You may follow the instructions below, or contact NAUI Pacific Rim at telephone number or registration@nauipacific.com for the Replacement. ALL requests for replacement cards must be in writing. Submitting a request for a replacement card does NOT guarantee that you will receive one.
To obtain a replacement card, send or email a clear photocopy of: your existing card (if possible), your training logbook pages, or your temporary certification card. Include these along with the Replacement C-Card Request Form (approx. 70 k) with a color photo. The form is in PDF format. You will need to download the Adobe Acrobat Reader (free) from Adobe to be able to read and print the form. If this does not work, you may obtain a form, by contacting NAUI Pacific Rim directly We accept most major credit card. Replacement cards are RM100 (Ringgit Malaysia). In summary, the steps are:
- Copy either your Temporary Card, Training Logbook Pages, your Original Card, or any other supporting documentation. Attach with the replacement request form.
- Scanning your photo or taking your photo with a digital camera and directly downloading it into a digital/jpeg file or with details of you names, and Download the Replacement C-Card Request Form (approx 70k). Print this form and fill it out.
- Fax or email from the steps 1-2 above to NAUI Pacific Rim with payment.
OnlyCredit Card orders are accepted via Fax. We do not accept orders via email at this time.
